Vocational Case Manager
JOB DESCRIPTION
Executes responsibilities in accordance with the organization's policies, procedures and applicable laws. Employees in this position direct and assist consumers with planning and managing their individualized program to assure it meets their needs in areas of vocational, residential, financial, personal and social areas in community based settings. Responsibilities include training, planning, assigning , directing work, addressing complaints and resolving problems as related to consumer needs. Will be responsible to train coaching staff according to consumer support plans.
Supervisory (Responsibilities)
Directly supervises job coaches as related to consumer support plans and employment sites and community integration. Will cross train on other Employment Training Specialists case loads to be able to provide coverage as necessary.
Tasks
- Collaborate with consumer and community resources to develop programs that address the needs of the consumer.
- Develop and coordinate programs for consumer to include job modifications ands strategies, task analysis and skill development.
- Promote and initiate integration, independence and natural supports in all support plans.
- Develop program plans , fade plans for consumer or assist in plan development as required by organization or provider guidelines .
- Participate in consumer case reviews.
- Provide requested information on consumers' progress and the development of case plans. Includes annual and semi annual plans.
- Conduct regular communication with Job Coaches as related to employment support.
- Observe and assess job coach's performance in the field to ensure optimal performance. Communicate concerns and training needs to the Associate Director.
- Confer with identified team members to better understand the backgrounds and needs of consumers.

Employee occasionally will bend, twist, lift or push up to 150 pounds; Employee must be able to lift a minimum of 50 pounds. Must be able to assist with wheelchairs, walkers, gait belts, and other adaptive equipment with respect to consumer needs. Must be able to transfer consumers with gait belts after proper training.

Our team is growing and we are currently looking for a
Job Coach
The Job Coach should have experience working with individuals with Traumatic Brain Injury, Serious Mental illness, and other disabilities. The Job Coach will report directly to the Associate Director or Executive Director. The Job Coach will be responsible for assisting consumers in learning job tasks, maintaining, their competitive employment skills and communication, and promote vocational and social independence in community-based job sites.
Job Duties and Responsibilities
- Will become fully acquainted with and adhere to the philosophy, agency policies, procedures and operation of the agency through:
- Orientation and in-service training
- Conference with the Executive Director, Associate Director and Vocational Case Managers
- Cross training at community job sites
- Reference materials provided by the agency
- Will train and supervise consumers with disabilities at community employment sites.
- Will assist in planning and carry out of all vocational activities as assigned.
- Regularly observe and record each consumer’s performance.
- Attend and participate in consumer meetings as assigned.
- Relay to all staff involved with the consumer any significant behavioral or functional changes.
- Work with the Vocational Case Manager to implement all program requirements.
- Will provide accurate information to assigned case managers regarding activities, consumer issues, employer issues, and needs of consumer.
- Will complete daily case notes, time sheets, and mileage sheets
- Will promote integration and independence at job sites and in the community according to supported employment guidelines.
- Will complete and update consumer's job site information sheets as assigned by the Associate Director.
- Will train consumers at specific job sites, utilizing appropriate teaching methodologies, task analysis, and job modification strategies.
- Will implement job modifications and fading plans with the assigned Vocational Case Manager to attain the most independent setting for consumers.
- Will advocate for consumers and others with disabilities in the community.
- Will attend staff meetings, agency training opportunities and review pertinent written materials as presented and assigned by the Associate Director.
- Will ensure the safety of consumers served through ongoing monitoring and assessment and ensure safe work habits in all work sites.
- Will assist consumers in meeting short term and long-term goals and objectives as identified in service plans.
- Will know all consumers’ transportation providers and schedules and how to contact them as needed.
- Will develop natural supports into the work site
- Will complete task analysis as needed for new sites, including time studies and competitive work analysis.
- Will independently set up all aspects of a new job site for consumers
- Will assist in the cross training of new Job Coaches at assigned job sites
- Will report all injuries to the Associate Director and Executive Director
- Will always maintain confidentiality.
- Will treat all consumers and all their team members with respect and courtesy.
- Will represent Work Plus in a professional manner.
- May be required to work evenings, holidays, and weekends.
- Transportation of consumers is required
- Make sure consumers are wearing seatbelts when in staff vehicles
- Transport consumers to and from the agency or work site in a safe manner
- Maintain a safe vehicle, monitor safety or personal contents in vehicle.
- Driving between sites is required

Required Qualifications
- Maintain a valid Wisconsin Driver’s License and clean driving record. Access to a vehicle for work purposes, good driving record, and proof of insurance.
- No record of abuse/neglect, or any other offense that would bar you from working in the human service field.
- 18 years or age or older. Will need to complete a criminal background check, driver's license check, and auto insurance check prior to employment.
- Background and driver's license check will be completed annually thereafter.
- Random Drug screens required for reasonable suspicion, or if involved in a motor vehicle accident while performing job
